Continued with cleanups:

- removed some empty lines at bottom of files
- added new-line character at end of file (as this else may confuse some old
  editors)
- moved JavaScript files out of "include" into own "js"
- if you set <basename>, you don't need full URLs in linked JavaScript files
  and CSS files
