Dynamic config loading

- Move settings, defaults and dbstructure to directory 'static'
- Dynamic loading of config files (after the static loading)
- Filter out '-sample.config.php' and '-sample.ini.php' files
- Remove unnecessary ConfigFileManager
- Move ConfigFileLoader to Utils
- Add tests for multi-loading for INI, config and sample-filtering

changed from http://www.gravatar.com to https://secure.gravatar.com . This should have been default so that any friendica site using ssl would not have to edit this to have a completed ssl setup.

Add check if libravatar addon is enabled, too.

Added a check to gravatar if the libravatar addon is also enabled.
Only one of these two addons should be anabled and libravatar will fall back to gravatar if it found nothing at libravatar.

Add Gravatar plugin.

This plugin provides the default gravatar lookup feature that has been in friendica's core before.
In addition it lets you choose the default avatar image type and select the allowed image rating.
